Background
The follow-up of children with congenital solitary kidney (SK) is controversial. Recent studies suggest that renal dysfunction may occur in the first decade of life. The aim of the study was to compare blood pressure, renal function and renal longitudinal diameter in children with SK with a control group (CG) of children aged 2, 5 and 10 years old.
Methods
Descriptive retrospective analysis of children with congenital SK, confirmed by scintilogram, followed at Paediatric Nephrology-Centro Hospitalar do Porto, from 1992 to 2010. CG composed of children with a history of acute pyelonephritis, with normal ultrasound, differential function ≥ 45% and without vesicoureteral reflux. Parameters analysed: blood pressure, serum creatinine, creatinine clearance (Schwartz formula), microalbuminuria and renal longitudinal diameter.
Results
The study included 40 children with congenital SK and 29 in the CG. The gender distribution and height were homogeneous in both groups. Children with SK presented significantly higher BP at the age of 5 for systolic blood pressure (p=0.047). The serum creatinine was higher in the group with SK, while creatinine clearance was lower, significantly at the age of 2 (p=0.001). identically, microalbuminuria was higher than in the CG, significantly at the age of 2 (p=0.019). Children with SK had a significantly higher renal longitudinal diameter than the CG (p=0.000) in the various moments of evaluation.
Conclusions
Children with congenital SK present blood pressure values, serum levels of creatinine and kidney sizes that tends to be higher, and conversely, a creatinine clearance that tends to be lower. Thus, it seems justified to follow-up children with solitary kidney.
