Isolated vascular rejection in the absence of cellular or antibody-mediated rejection is an uncommon finding. Reports of this phenomenon in the pediatric population are even more rare. There is no consensus on the significance of, or the necessary treatment for, an isolated vascular rejection in renal transplantation. We present the case of a adolescent with end stage renal disease secondary to Wegener's granulomatosis found to have an isolated vascular rejection in only one vessel on a protocol renal biopsy and discuss the available literature.Keywords
