Abstract

The study tried to comparatively analyze surgical findings based on the examination results of polysomnography and
video-fluoroscopy which are examinations useful in diagnosis of obstructive sleep apnea syndrome. The subjects
were patients over the age of 18 who were admitted for snoring, observed apnea, or sleepiness. Received under
polysomnography and video-fluoroscopy inspection of obstructive sleep apnea of doubt from March 2013 to February
2014, it was intended for patients who have undergone surgical treatment. In the analysis was obstructive sleep apnea
severity, there were difference in all clauses excluding age and height (P<0.05). Crosstab due to the presence or absence
of video-fluoroscopy and polysomnography and surgical treatment, there was a significant difference (p<0.05). There are
many examinations for diagnosing obstructive sleep apnea but as an essential examination there are polysomnography
and video-fluoroscopy examination that can observe the dynamic change of the obstruction which are clinically useful.

Abstract

Objectives: In the radiation therapy of cancer that occurs in hepatobiliary system, we want to compare and study the usefulness of the method to minimize movement of tumor by using artificially respiratory control device (Abdominal Compressor: AC) and the inducing respiratory control equipment (Abches System: AS). Methods/Statistical Analysis: For the purpose of the present study, analysis is conducted by acquiring images of free breathing(FB), artificially AC and inducing AS respectively of 30 patients. Findings:Results of the analysis to the subjects in 3 specific groups Analysis results of 3 patients groups according to the movement of tumor controlled by three methods is the following. Average ITV and PTV are reduced in AC and AS group by 42.0% and 32.6% respectively. In reproducibility, the variation values are lateral (RL) direction (X) -0.28 ± 0.65mm, vertical (SI) direction (Y) 0.02 ± 1.22mm, and (AP) direction (Z) 0.29 ± 1.55mm. Among the three-target-groups of patients, the maximum error occurs in FS which are side (RL) direction (X) -0.77 ± 1.65mm, vertical (SI) direction (Y) 0.05 ± 3.4mm, and (AP) direction (Z) 5.7 ± 2.86mm so that AC and AS group have less error and better reproducibility. Statistical analysis on tumor volume (ITV,PTV) of 3 specific group shows reduction of volume in respiratory control group. The most reduction is in AC apply from 1468.3±446.8 cc in FB to 757.7±210.3 cc in AC((p<0.00). Application/Improvements: In conclusion, radiation therapy is to protect normal tissues, with the aim of suppressing tumors so the motion associated with respiration during radiation therapy is minimized. Fractionated radiation therapy and high dose 1 fraction radiation therapy demands great reproducibility and appropriate respiratory control for site- specific tumor needs to be considered.

Abstract

Objects: This study aims to find a way to control infection of mammography effectively, by culture identification of the microbialflora in hospitals. And, it will suggest importance of infection management and necessity of education for workers and patients. Methods/Statistical Analysis: The way of collecting clinical material was done by keyboard and mouse being used by most of workers for mammography among 10 medical institutions using cotton swabs as transport medium on contacted skin part of patient and wiping out top, middle and bottom of it. Transport medium used after collecting examining materials was put and sealed with tip dipped in an individual culture medium (10cc). Sealed materials are assorted by examination and used within 24 hours. Findings: When examining the distribution of detected bacteria, Staphylococcus epidermidis comprised 6 cases, 15% of the total, while Enterococcus faecalis was detected in 4 cases, 10%, each of I, and Serratiamarcescens, occurred in 3 cases, 7.5%, each of Enterococcus faecium, and Gram negative bacilli occurred in 2 cases, 5%, Providenciarettgeri, Klebsiella pneumonia, and Micrococcus species occurred in 1 case, 2.5% percent. There are cases where bacteria did not grow after culture for 24 hours at 37°C, accounted for 42.5% of 17 cases, and bacteria were not detected in a significant proportion of locations tested. Among the specimens collected, the lowest amount of bacteria was found on the Face block. Also, while the Staphylococcus epidermidis, which was found at the highest levels, was detected on none of the compression paddles, it was detected most frequently on the grid. Application/ Improvements: Especially, detection of contamination materials in breast imaging room that makes patient’s skin straight exposed and has highest contact frequency between examiners and patients. Will be expected to contribute to high sense of sterilization of examiners and much more putting priority on prevention of infection.

Abstract

Objective: This reserach was conducted to review whether there was a difference in the method of attempted suicide in patients admitted to the emergency medical center at S region, from January 2014 to December 2014. Methods: We reviewed retrospectively the medical records by using 162 cases of attempted suicide patients in the emergency medical center. An analysis of medical treatment result based on gender and age characteristics, season of suicide occurrence,suicide attempt method, and treatment result based on suicide attempt, was conducted by investigating general characteristics, season of suicide occurrence, emergency medical care, and suicide attempt methods. Findings: As per the result of the analysis, suicide by consuming sleeping pills was the most common among the suicide attempt methods. Hospital transfer due to consumption of sleeping pills and mortality rate due to pesticide poisoning were the highest. There was no difference across the time of arrival at the hospital, the season or the day of the week. In conclusion, emotional support from family members is desperately required to prevent suicides via hanging that have the highest effect on death and to better prevent the usage of medications meant to be used as sleeping aids as an aid to suicide. Improvements: Therefore, based on the results of this research, we intend to develop a suicide prevention program suitable for age group, and provide basic information necessary for establishing a national health project plan that reduces public health expenses.

Abstract

Background/Objects: The purpose of this study is to compare a representative position and two positions what are used clinically at Chest PA examination. Methods/Statistical Analysis: The first position is generally known for minimizing overlap of lung fields and scapulae, and the other 2 positions are mainly used in actual clinics. 3 Radiologic technologists with an average of 7 years' experienced reviewed the images. The overlapping area was measured by connecting the overlapping part of the lung field and the scapula in the radiograph with dots and lines. Findings: We compared the incidence of overlap of scapulae and lung fields depending on sex and age. There were no statistically significant differences between males and females in CP (p>0.05). However, in IRP and HP, there were meaningful differences in the incidence of overlap depending on sex (p0.05). CP showed the least overlapping area followed by HP and IRP. Regarding the ratio of overlapping area, CP had the highest ratio and IRP had the least ratio, but this difference was not statistically meaningful according to Kruskal-Wallis H examination. For reference, a value closer to 1 in this test indicates less of an overlap between the scapula and lung field. Therefore, a position with a value closer to 1 is more useful in separating the scapula and the lung field when conducting Chest PA. Applications/Improvement: The research revealed that both positions are useful in minimizing the overlap between the lung field and scapula.

Abstract

Background/Objectives: When receiving radiation treatment in form of tomotherapy, most head and neck cancer patients show symptoms of weight loss caused by stomatitis and esophagitis, and skin burns. Weight loss may lead to a closer distance between the skin and the target, causing a change in the measured skin dose. This study was conducted to analyze such changes during the radiation treatment period. Methods/Statistical Analysis: The study also analyzed whether factors such as patient gender, age, diagnosis type, and treatment regimen, including surgery or chemotherapy in addition to radiotherapy, might influence the weight changes in patients, and collected values for changes in skin dose using thermoluminescence dosimeters. Findings: The study considered 20 patients with head and neck cancer, and all of these patients showed weight loss and an increase in measured skin dose. In addition, the gender or age of the patients had no direct relationship with their weight changes. Patients who hadn't undergone surgery prior to concurrent chemotherapy and radiation therapy showed a substantial weight loss. In comparison, the patients who had undergone surgery prior to concurrent chemotherapy and radiation therapy displayed the least amount of weight loss. As the amount of weight loss became larger, so did the skin dose that was measured during tomotherapy (p<.05). Application/Improvements: It appeared that greater weight loss led to an increase in measured skin dose, From this study, it is implied patients with large weight loss may benefit with an adjustment of their radiotherapy parameters while maintaining their target doses.
