In the recent technology, NFC (Near Field Communication) is a recently growing technology for communication for short range which is aimed to augment existing near field technologies such as RFID (Radio Frequency Identification). In a health care, the use of NFC (Near Field Communication) technology can be employed for not only bringing down health care costs but also facilitate smooth-running and automation of patient identification processes in hospitals and use of mobile devices like smart phones, PDA etc. for designing of health care management systems. In this technology enabled world, changes are rapid and are constantly disrupted. One such disruption now-a-days is Internet of Things (IoT), that has the potential to change the way healthcare is delivered. Considering these factors, a system is proposed that monitors the patient’s condition through a smart android phone associated to the particular patient. Also the doctor will have an NFC enabled smart phone associated where by swapping the NFC tag, the unique id of the patient will be obtained and the doctor can view the status and the history of the particular patient. This proposed system reduces the time and paper work required during the registration of the patient and also the disadvantages of RFID based system is overcome by the use of NFC tags.
