FAO/WHO/UNU recommended 1.2 MJ/d extra energy during pregnancy for women who maintained their pre pregnant level of activity. This amounts to 335 MJ for 40 weeks of gestation. This figure is reduced to 235 MJ (0.84 MJ/d) for those who reduce their physical activity below pre pregnant levels. These estimates are based on theoretical costs of protein and fat accumulation in the mother and fetus and for the maintenance of the new tissues. Regeneration of proteins, either in liver or in plasma and increase in body weight following protein repletion, have been used as indices to evaluate the quality of protein. Changes in the nutritional state of animal influence the activities of enzyme systems in different organs. Liver loses labile proteins rapidly on protein deficient diets and as a result, serum albumin gets decreased. These conditions are reversible with refeeding. The study was aimed to assess the macronutrient intake pattern and its association to glycemic status and drug intake pattern of type 2 diabetes patients. Forty five subjects (25 males and 20 females) aged between 30 and 60 years, diagnosed with diabetes for less than 3 years were enrolled into the study. Based on Oral Anti-Diabetic drugs (OAD) treatment, they were grouped as those on mono-therapy (GpI, n=21) and dual therapy (GpII, n=25). Information about drugs profile, macronutrient intake (3 days diet recall reported as % equivalent of energy) and serum glucose profile were obtained at baseline and a 6th month follow up. Mean CHO, protein and fat intakes were 65.4±4.84, 12.6±1.26 and 22.0±4.7 % of total energy at baseline and remained essentially similar at 6th month. Patients who had controlled glycemic status (HbA1C ≤ 7 %) consumed 63.4±2.70 % CHO which was significantly (P<0.05) lower than those with not-controlled glycemic status (HbA1C >7 %) during both the assessment periods. Also, mean protein intake was higher and fat intake was lower among patients with good glycemic control. Significantly a higher proportion of (P<0.05) patients consuming CHO > 65 % were on dual therapy. A diet with lower carbohydrates and fat but high protein exerted good glycemic control. Number of OADs and their dosages were significantly associated to CHO intakes. The study was aimed to assess the serum vitamin B12 status of diabetic patients managed on metformin mono or combination therapy. A total of 45 patients (25 males; 20 females) diagnosed with type 2 diabetes for less than 3 years were enrolled into the study. Two groups were formed - metformin monotherapy or Gp1 (21 patients, mean age = 42.8 ± 6.82 years) and combination therapy group or Gp2 (24 patients, mean age = 42.6 ± 7.05 years). It was a self-control study wherein their serum vitamin B12 was measured at baseline (initial) and after 6 months (final). Demographic profile was obtained at baseline. Dietary intake of vitamin B12 was assessed by 3 days recall, while clinical manifestations of B12 deficiency and metformin dosages were obtained at initial and final period. Mean dietary intake of vitamin B12 was 1.16 ± 1.01 μg/day at baseline and 1.25 ± 1.15 μg/day at the sixth month, in the two groups. The mean serum B12 levels of group 1 and 2 at baseline were 347.54 ± 146.10, 293.47 ± 90.90 pg/ml, respectively and those at sixth month were 296.24 ± 144.45 and 239.32 ± 89.90 pg/ml respectively. The mean reduction in serum B12 was 51 and 54 pg/ml in Gp1 and Gp2 respectively, during the six-month observation period and the difference was statistically significant (P = Gp-1; 0.001, Gp-2; 0.002). An inverse relation was found between serum B12 and dosage of metformin. The proportion of patients with weakness, tingling/numbness in hands/feet increased markedly in the sixth month in both the groups. Patients on metformin, both alone or in combination, need monitoring and appropriate management of B12 deficiency.Keywords
