- Comparision of Bupivacaine alone and its Combination with Different Doses of Fentanyl in Spinal Anesthesia for Cesarean Section: a Prospective Randomized Study

Background: Intrathecal administration of local anesthetics alone does not provide adequate analgesia during caesarian sections. The visceral pain associated with peritoneal traction can be obliterated by the addition of intrathecal opioid, due to their synergism.
Aims: The aim of this study was to observe the effect of intrathecal bupivacaine alone and in combination with fentanyl in two different doses.
Settings and Design: We performed a prospective randomized study on 90 consecutive patients undergoing caesarian section in our hospital.
Methods and Material: Ninety selected patients, who underwent cesarean section, of age ranging from 20-35 years, were randomized into three equal groups. Group I received hyperbaric bupivacaine (B) alone, where as group II and group III received hyperbaric bupivacaine along with 12.5 μg and 25 μg of fentanyl respectively (BFL&BFH). The quality and durations of analgesia was compared between the three groups. The effects of drug on neonate were monitored on the basis of Apgar score.
Statistical analysis: Unpaired't' test&'Z' test were used for statistical analysis.
Results: There were no differences in regard to Apgar scores between all three groups. The synergistic advantages were evident in both combination groups with respect to patient's comfort and ease of surgeons; but the maternal side effects were more in the group with higher dose of fentanyl (BFH). No adverse effect on neonates was observed in all three groups.
Conclusion: Since the clinical dose of spinal bupivacaine required to obliterate visceral pain causes undesirable side effects. Addition of fentanyl with bupivacaine, reduces the dose of local anesthetics in achieving adequate surgical anesthesia. We conclude that the combination of 12.5 μg of fentanyl along with 2ml of 0.5% hyperbaric bupivacaine achieved adequate anesthesia with minimal maternal and neonatal adverse effects.

Background: The LMA Supreme (LMA) and the i-gel are two supra-glottic airway devices, which offer potential benefits in general anaesthesia.
Aims: The aim of this study was to observe benefits of LMA Supreme over i-gel for ventilation in patients undergoing laparoscopic surgery.
Settings and Design: We performed a prospective randomized study on 100 consecutive patients undergoing laparoscopic surgery in our hospital.
Patients and Method: Hundred selected patients, who underwent laparoscopic surgery, of age ranging from 20-55 years, were randomized into two equal groups. In the group I (n=50) LMA SUPREME (No.3 & 4) and group II (n=50) i-gel (No. 3 & 4) were introduced for ventilation during general anaesthesia. In this study, we compared the insertion success rate and ventilation profile between the LMA Supreme and the i-gel groups undergoing laparoscopic surgery. The first-time success rate, insertion time, ease of insertion of gastric tube and complications were analysed.
Statistical analysis: Unpaired 't' test & 'Z' test were used for statistical analysis.
Results: We found better first time success rate, and fewer failures with the LMA Supreme compared with the i-gel, indicating that the LMA Supreme may be preferable for mechanical ventilation in laparoscopic surgeries.
Conclusion: LMA Supreme has an edge over the i-gel with respect to ease of insertion of device and gastric tube with minimal complication in mechanical ventilation for routine laparoscopic surgery.

Intraocular pressure is the pressure exerted by the contents of the eye against its containing wall. During general anaesthesia, the intraocular pressure can increase, rendering problems in ophthalmic surgery especially surgery on the eye bulb. Pre-treatment with non-depolarising muscle relaxant prevents fasciculation and so the intraocular pressure does not rise. This is a prospective randomized study of 100 patients divided into two equal groups, where we have compared the effect of vecuronium bromide and atracurium besylate on intraocular pressure. The intraocular pressure was measured before surgery, at the time of intubation and at every 10 minutes interval during surgery. All the data were analysed statistically using unpaired student t test. We concluded that vecuronium bromide has better control over intraocular pressure as compared to atracurium besylate.Keywords

Background
Cesarean section(CS) is the most commonly performed obstetric surgical procedure throughout the world. Usually this is a life saving procedure for both mother as well as new born baby, but it also have some complications, related to both surgical and anaesthetical.
Aims & objectives
To evaluate different complications encountered during and after cesarean section within a period of 10 years at LSRH&SIMS and its management.
Materials & methods
An analysis of 8550 patients was done at LSRH&SIMS during 2000-2009. The patients who underwent cesarean section whether it was elective or emergency, cases booked or unbooked, done for various indications constituted the subject of study. The patients were analysed with respect to types of maternal morbidity encountered.
Results
In our study, we analysed 8550 deliveries during 2000 to2009, out of which 22% of patients were subjected for CS. Out of 22% of CS, 86.6% patients were operated as emergency and 13.3% as elective section. In present study, minor&major morbidity were seen in 33.3% of cases. Rest of the patients recovered without any problem in postoperative period.
Conclusion
In present study, incidence of CS was 22%. Most common indication for CS was foetal distress, followed by previous CS, prolonged labour, malpresentation&APH, etc. Incidence of maternal morbidity in this study was 33.3 %.
