It could be said that the most serious and important social and ethical problems facing medical education are those that originate in the transformations that medical schools and academic health centers have weathered throughout the last four decades. The long-standing tendency of medical educators to attach “magic-bullet” significance to the power of designated courses to positively influence and professionally shape the attitudes and behavior of medical students and young physicians seems unduly optimistic, and somewhat misdirected.

Hardly surprising as most of them, myself included, were not adequately equipped with genetic insights into the development, treatment, and prognosis of illness. Part of this reluctance I believe is because of the inherent aversion among clinicians to keep themselves sufficiently abreast with advances in basic sciences. There is an ongoing debate in progress as to the relative merits of a primary/preventive focussed healthcare system versus a healthcare system where the major emphasis is on developing tertiary healthcare needs, with primary and preventive healthcare relegated into background. Each system has its fervent advocates. While we can accept that one model of healthcare may not be suited for all the countries with their differing needs and vastly different sociocultural contexts, it would be instructive to learn a few lessons from one country which has made excellent strides in its healthcare statistics, but with almost an exclusive emphasis on primary and preventive healthcare.Keywords

We have just gone through the yearly ritual of enunciating the special needs of the mentally ill on World Mental Health Day. Years of campaigning by human rights advocates have prompted the United Nations to recognize the human rights of the mentally ill through a resolution by the United Nations Human Rights Council. However, the sad fact remains that most coun-tries in the world have not even seen fit to ratify this resolution, let alone treat it with the degree of urgency it clearly merits. The plight of the mentally ill is a matter of collective shame for the entire humankind. But the platitudes have not translated into visible benefits that have percolated down to the ones who need them. Problems also persists because of faulty plan-ning and overall apathy. These have to be addressed head-on if we truly mean to improve the lot of the most disadvantaged among us.Keywords
