Aortic aneurysm causing bone erosion is uncommon and is rarely associated with neurological deficits. Only few cases have been reported. We report a case of atherosclerotic aneurysm of the descending thoracic aorta causing extensive erosion of vertebral bodies T5-8, leading to invasion of the spinal canal and compression of the cord. The clinical signs were chronic thoracolumbar pain and spastic paraparesis. Clinicians must remain cognizant while evaluating a case of spastic paraparesis about the possibility of aortic aneurysm, a potentially treatable cause.
